Vertical dimension control, asymetrics, clinical guidance for mini implant use

Samenvatting

 

1. Vertikale dimentie contrôle  

Orthodontische behandelingen werden uitgevoerd volgens dezelfde standaards en modaliteiten voor patienten met verschillende caracteristieken inlusief vertikale dimentie.

Later werd de belangrijkheid van de vertikale dimentie onderlijnd, in het bijzonder bij patienten in volle maturiteit, wordt de behandeling meer en meer individueel.

Tijdens de adolescentie, de wijzigingen welke het complex van de maxillomandibular beinvloeden, is het resultaat van de groei van verschillende onderdelen in de 3 dimenties.

Behandeling van de vertikale op dit ogenblik is moeilijk, door het feit van zijn interactie met de sagittale en de dwarsafmetingen.       

Op basis van bepaalde therapeutische concepten, moet onze behandelings planning verschillende voorspellende parameters in acht nemen, zoals faciale groeipatroon, occlusievlak helling, smile line situatie teneinde een individueel behandelingsobjektief in te plannen met het oog op een best mogelijk estetisch resultaat.

 

2. Asymmetrieen in orthodontie

Asymmetrieen worden beschouwd als een van de moeilijkste te beheren situaties in de orthodontie.

Een correcte differentiale diagnose vormt de hoeksteen voor een biomechanische toepassing.

De asymmetrien zouden geklasserd kunnen worden als posterieure en voorste afhankelijk van hun positie.

Hun orientaties volgen het occlusale plan, het sagittal plan en het frontaal plan et hun etiologie kan tandkundig, funktioneel of skeletaal zijn.

Anterior asymetrie zijn gewoonlijk moeilijker te behandelen door hun interactie met de glimlach esthetiek.

Posterieure asymetrie, zijn discreter, maar dienen echter beschouwd te worden als een intermaxillaire oclusale relatie, met het zicht op bekomen van exellentie in orthodontie.

Het doel van deze presentatie is het identificieren van de verschillende types van asymentrie, van het ontwikkelen van verschillende diagnoses en een mechanische behandeling toe te passen afhankelijk van ieder geval.

 

3. Klinische richtlijnen voor het gebruik van Mini-implants.

Tijdens deze presentatie, zullen wij ons focussen op de gevolgde logica van onze keuze van mini-implants, de wijze van plaatsing en hoe onze orthodontische kracht toe te passen

Klinische raadgevingen zullen besproken worden teneinde complicaties te vermijden.

 

4. Mini-implants in Orthodontie : Therapeutische concepten en actuele technieken.

Orthodontie wordt beheerst door behandelings concepten afhankelijk van onze orthodonte filosofie.

De komst van Mini-implants bracht een mechanische revolutie in de orthodontie, en stuurde ons naar de herziening van onze denkwijze.

Het gebruik van Mini-implants is op basis van principes die dienen eerbiedigd te worden teneinde, wil men concluderende resultaten bekomen .

Deze principes houden eveneens in, de keuze van de types mini-implants alsook richtlijnen en plaatsen.

De doelstelling van deze presentatie is aan te tonen hoe we deze aanpassingen zouden kunnen toepassen in onze moderne orthodonte praktijk.

• Vertical Dimension Control

 

Orthodontic treatment has been performed according to the same modalities and standards for patients presenting different characteristics including vertical facial type. Later on, the importance of the vertical dimension has been pointed out, especially in growing patients, and treatment approach became more and more individualized.

In adolescence, modifications affecting the maxillo-mandibular complex are the result of growth of different components in the three dimensions. Then, treating separately the vertical is difficult due to its interaction with the sagittal and the transverse dimensions.

Based on some therapeutic concepts, our treatment planning has to consider different prognostic parameters as facial growth pattern, occlusal plane inclination and smile line situation in order to set up individualized treatment objectives and attain the best final esthetic result.

 

• Asymmetries in Orthodontics

 

Asymmetries are considered as one of the most difficult situations to manage in orthodontics. A correct differential diagnosis constitute the corner stone for any biomechanical application.

These asymmetries could be classified in posterior and anterior depending on their location. Their orientation follows the occlusal plane, the sagittal plane and the frontal plane and their etiology could be dental, functional or skeletal.

Anterior asymmetries are generally more difficult to treat due to their interaction with smile esthetics. Posterior asymmetries, more discrete, has to be also considered for a better occlusal intermaxillary relationship and in the pursuit of excellence in orthodontics.

The aim of this presentation is to identify different types of asymmetries, to develop a differential diagnosis and to apply an individualized treatment mechanics depending on every case.

         During this presentation, we will focus on the logic followed to choose a mini-implant, the mode of placement and how to apply and adjust our orthodontic force. Clinical tips will be discussed in order to avoid any complications.
